As we continue our efforts to better serve our customers and vendors, we have recently enhanced our vendor qualification process. Please complete this Vendor Profile and submit copies of the following documents:

• Hazardous Certification.
• Insurance Certificate (Intermodal Management System, LLC must appear as certificate holder).
• Minimum insurance requirements include:
  Cargo Liability- $100,000 per vehicle.
  Automobile Liability- $1,000,000 per occurrence.
• Operating Authority.
• Signed W-9.
• Signed "Agreement for Motor Transportation" .
