- Hazardous Certificate.
- Insurance Certificate (Intermodal Management System, LLC must appear as certificate holder).
Minimum insurance requirements include:
- Cargo Liability - $100,000 per vehicle.
- Combined Automobile Liability - $1,000,000 per occurrence.
- Copy of Worker's Comp Insurance.
- Operating Authority.
- Signed "W-9"

- Signed "Agreement for Motor Transportation".
